Q:  What are your office hours?

A:  Our offices are open from 8 AM to 5 PM Monday through Thursday and 8 AM to 12 PM (noon) on Friday.

Q:  How do I scheduled an appointment ?

Q:  How long will my first appointment last?

​

A:  Your first appointment may take up to an hour.  Please keep in mind that patient emergencies may occur and your physician may be late to your appointment. If this occurs, we ask for your understanding and patience.

Q:  What should I bring to my appointment?

​

A:  Please bring the following:

  • All medications and their original containers with their labels

  • Co-payments

  • Insurance cards

  • Medical records, if applicable

  • Primary care physician authorization

  • Other information that is pertinent to your visit

 

If you are a first-time patient, please arrive early to complete patient registration forms if you have not already done so.

Q:  If I call the office to speak with the nurse, what type of information should I leave on the voicemail?

​

A:   Please speak clearly, sharing:

  • Your first and last name

  • Your date of birth

  • Your phone number

  • The reason for your call  

 

If you need a refill on a prescription, please provide us with the name of the medication and your preferred pharmacy.

Q:  How soon can I expect a return phone call?

​

A:  We strive to return the phone calls within 24 hours.

Q:  When I call to request a prescription refill, when will the prescription be relayed to my pharmacy?

​

A:  Prescription refills are called into the pharmacies at the end of each day.

​

​

Q:  What if I need to cancel an appointment?

​

A:  If you must cancel the appointment, please call our office as soon as possible so that we can best serve you and our other patients.

Q:  Do you accept Medicare?

​

A:  Louisiana Cardiovascular and Limb Salvage Center is a participating Medicare provider.
