Q. :  What are your office hours?

A  :  Our offices are open from 8 AM to 5 PM Monday through Thursday and 8 AM to 12 PM (noon) on Friday.

 

Q: How do I scheduled an appointment ?

A: Please call one of our offices to scheduled appointment.  Normally patients have seen by appointment only, but we accept walk-in patients as well.

Q: How long will my first appointment last?

A: The first appointment may take up to an hour.  When emergencies occur, your doctor may run late and, if so, we will ask for your understanding and patience.

Q: What should I bring to my appointment?

A: Please bring the following:

  • All medications and their original containers with their labels

  • Co-payments

  • Insurance cards

  • Medical records, if applicable

  • Primary care physician authorization

  • Other information that is pertinent to your visit

 

If you're first-time patient, please arrive early to complete patient registration forms if you have not already completed them prior to visit.

 

Q: If I call the office, needing to speak with the nurse, what type of information should I leave on the voicemail?

A:   Please speak clearly, sharing:

  • Your first and last name

  • Your date of birth

  • Your phone number

  • The reason for your call  

 

If you need a refill on a prescription, you should provide us with name of the medication, number of pills that he is supposed to take each day, the number of times and the number of times each day that you should take the medication.  Additionally, please leave the name of the pharmacy and the location.

Q: At Louisiana cardiovascular limb salvage center, how soon can I expect a return phone call?

A: We strive to return the phone calls within 24 hours. It will be of incredible help to our staff if you could kindly leave a detailed message with all the information (see the question above for more details).

Q: When I call to request a prescription refill, how will the prescription will be relayed in to my pharmacy?

A: Prescription refills are called into the pharmacies at the end of each day.

Q: What if I need to cancel an appointment?

A: If you must cancel the appointment, please call our office as soon as possible so that we can best serve you and our other patients.

Q: Do you accept Medicare?

A: Louisiana Cardiovascular and Limb Salvage Center is a participating Medicare provider, and we accept assignment of all Medicare benefits.  As a courtesy, we file your supplementary insurance.
